The CCM services program pays around $500 a year a patient when these services are constantly performed monthly. One the biggest problems the physicians are facing during implementing Chronic Care Management Services is the incapability of their software applications, which includes EHRs, to keep track of the time spent interacting with patients through non face to face interactions. CCM was architected to facilitate the process, which would allow highly efficient and organized documentation, tracking of time and optimum usage of staff member time. CCM also assists you manage all aspects of the CCM program, which includes compliance with Medical requirements.

    Chronic diseases such as heart diseases, diabetes, obesity, mental health, hypertension, COPD and congestive heart failure are one of the most treatable and common health problems in the USA. It is estimated that 66% of Medicare beneficiaries have either two or more chronic conditions. If 50% of the eligible patients received CCM services for at least a year, that would result in over $10 billion in Medicare payments on an additional basis, to provider organizations.

    ELECTRONIC HEALTH RECORD (EHR)

    Electronic Health Record (EHR) can help create robust data and information for better patient care plus health monitoring.

    There is a large growing interest in the usage of data captured in the Electronic Health Records for patient registries. Both the patient registries and EHRs capture and utilize clinical data but theoretically, they are designed for several purposes. An EHR is a digital system maintained and used by healthcare systems to accumulate and store patients’ clinical data or medical information. EHRs offer a varied range of data that can be merged, linked or integrated directly into a registry. The collection of a Common Clinical Data Set (CCDS), led by the Meaningful Use program was found across most providers. The data types which are commonly extracted from the EHRs and imported into the registries are demographics, patient identifiers, diagnoses, procedures, medications, laboratory results, utilization events and vital signs.

    Registries that apply EHR information may find usage in a varied range of IT system architectures. The architects of registry should consider the number of partaking sites, types of underlying EHRs, connectivity and existence to Health Information Exchanges (HIEs). Healthcare suppliers often develop, evolve and manage EHR- based registries that are utilized to support medical care and achieve operational targets. Clinical registries often develop the usage of centralized architecture and usually have an EHR information warehouse as their spinal cord, along with multiple information marts including varied ranges of registry data.
